Register at Harpers Ferry Family Medicine

171 Taylor Street, Bolivar

Thursday, August 5th at 7pm

Please pre-register at www.eayso.org.  Print and bring 2 copies of the player form with you. We will be registering all children ages 18 - 4 (DOB 8/1/91-7/31/2006). You must bring a birth certificate if your child has never played with JCYSL before. The registration fee is $85 for the full season.
