AYSO Region 322 - Jefferson County Youth Soccer

We are now first come first serve! We will be registering all children ages 18 - 4 (DOB 8/1/91-7/31/2006).

Print, sign, date, initial and mail 1 copy of each child's player form and mail to:

You will need to print and sign a second copy of each child's player form.  You will deliver this form to the coach on the first day of practice.

Please include the player fee of $85 per child for the full season, fall and spring. You will also need to send a copy of your child's birth certificate if this is their first time playing with JCYSL. Please include your child's age/weight on the player form when preregistering. It is used to order proper uniform sizes.

Please note that the player form is 2 pages -  the second page is the release that must be initialed and mailed. Please email the registrar at This e-mail address is being protected from spambots. You need JavaScript enabled to view it if you have any questions or problems.

We can not process your child's registration without all the required information mailed to JCYSL.
